Dementia: Mini Audit

Dementia contributes to significant loss of healthy years to Australians aged over 65 and is the leading cause of burden of disease in older Australians.  Correct identification of dementia and management based on clinical practice guidelines can help decrease the burden of disease and improve the lives of those affected and their carers and loved ones.

The Activity

You will be required to identify five patients aged over 65 who have received a dementia diagnosis and

• Reflect on
  • How you screened and diagnosed these patients i
  • What sceening tools and cognitive test were performed
  • If referral was required/made to assist with diagnosis
  • What management was implemented within the first year of diagnosis including pharmacological and non-pharmacological measures
• Reflect on your practice in comparison to the best practice guidelines
• identify areas for improvement in practice and formulate an action plan to address these areas.
