Sleep disturbance and insomnia are common problems seen in primary care. Many patients with sleep difficulties have underlying physical or psychological issues that need to be addressed, but in a large proportion no health based cause can be found. Whatever the initial reason for development of disturbed sleep it can be distressing and even disabling, and it can evolve to become the main problem in people’s lives.
The advent of effective and apparently side effect free hypnotics has been a boon for short term management of sleep disturbance, but long-term use can create significant problems with side effects, dependence, tolerance and continuing poor sleep quality.
