Conduct a comprehensive review of veteran patient records.

Develop and implement effective care plans for veterans.
Access and utilise DVA-funded services and supports.
Select two veteran patients from your practice with differing health profiles (e.g., a recent veteran with PTSD and an older veteran with chronic pain). Analyse how their military service may have influenced their current health needs.
Develop a targeted care plan for each veteran, addressing their specific health concerns. Ensure the plan includes strategies to optimise access to DVA entitlements and support services.
Evaluate the effectiveness of current management strategies for each veteran's health conditions. Identify any barriers to care and implement strategies to improve their wellbeing.
